CAT-TASTROPHE CHARITIES

Adoption Application

Thank you for wanting to adopt through Cat-Tastrophe Charities. Our goal is to place each cat or kitten in a safe, loving, and permanent home. Please complete this application fully and honestly.

Adoption Donation Policy

Cat-Tastrophe Charities requires a $500 adoption donation for each adopted cat or kitten. No cat or kitten will be released until the $500 adoption donation is paid in full.

This adoption donation helps cover veterinary care and rescue costs required to prepare cats for adoption, including vaccinations, spay or neuter, microchip, parasite prevention, testing, and other medical treatment as needed.

Because funds are used immediately to support rescue operations and medical care, the $500 adoption donation is non refundable for any reason, including surrender, return, change of circumstances, or if the adoption does not work out.

Cat-Tastrophe Charities is a small, foster based rescue run by one person. Every adoption directly supports the next cat in need.

By initialing below, I confirm I understand and agree to the $500 adoption donation requirement, the payment before release policy, and the non refundable policy.

Are you applying to adopt
Where did you meet or learn about this cat?

Applicant Information

Are you at least 21 years old?

If you rent, proof of landlord or property approval for a cat is required prior to adoption. This may include a written lease clause, email confirmation, or signed approval from the property owner or manager.

Is everyone in the household aware of and supportive of adopting a cat or kitten?
Does anyone in the household have allergies to cats?

Living Environment

Type of residence

Bonded Pairs and Companionship

Some cats and kittens are bonded, meaning they are emotionally attached and do best together. For bonded cats or bonded kittens, Cat-Tastrophe Charities requires they be adopted as a pair unless there is already a friendly cat or other compatible pet in the home for companionship. This policy helps prevent stress, loneliness, and behavioral issues and supports a smoother transition.

Are you willing to adopt a bonded pair if the cat or kitten you want is bonded?
Do you currently have another cat or compatible pet?

Current and Previous Pets

Do you currently have pets?
Are your current pets up to date on vaccinations?
Have you ever surrendered a pet to a shelter or rescue?

Commitment

Cats can live 15 to 20 years or longer. Are you prepared for this long term commitment?
Are you prepared to provide routine and emergency veterinary care if needed?

Final Agreement

I certify the information provided is true and complete. I understand that false information may result in denial of adoption. If the adoption is completed, I understand that violating the adoption agreement may result in Cat-Tastrophe Charities reclaiming the cat.

If I rent, I agree to provide proof that a cat is permitted in my residence before adoption is finalized.

I understand the $500 adoption donation is required before release and is non refundable, including in the event of surrender or return.

To support a safe and successful adoption, I understand Cat-Tastrophe Charities may verify the information provided, including contacting my veterinarian, landlord if applicable, and references. I understand a home visit or virtual home check may be requested prior to approval.
